Crystal Blue Disability Services is currently accepting suitable SIL and daily support referrals across Western Sydney and surrounding areas.
We are a registered NDIS provider and are now registered for 0138 Assistance with Supported Independent Living. We have successfully passed our mid-term audit and are ready to discuss suitable SIL-related referrals.
We support participants with daily living, personal care, community access, transport, skill-building and SIL-related supports.
Who can submit a referral?
- Support coordinators
- Participants
- Families or nominees
- Plan managers
- Allied health professionals
- Other professionals involved in the participant’s support
Information that helps us review a referral
- Participant name and suburb
- NDIS funding type
- Support needs and goals
- Preferred days, times or start timeframe
- Any relevant risks, behaviours of concern, mobility needs or communication needs
- Support coordinator or key contact details
